Is Your Leg Pain Coming from a Muscle or Your Spine?

Pain that radiates from the buttock down the leg is not always caused by a herniated lumbar disc. In some cases, it may be the result of Piriformis Syndrome, a condition in which the piriformis muscle compresses or irritates the sciatic nerve. This condition is commonly seen in runners, cyclists, office workers, and individuals who spend long periods sitting. Because its symptoms closely resemble sciatica caused by spinal disorders, an accurate assessment is essential for appropriate treatment.

The piriformis is a small but important muscle located deep within the buttock. It helps stabilize the hip joint and assists with hip rotation during walking, running, and other daily movements. In certain individuals, the sciatic nerve passes very close to—or even through—the piriformis muscle. When the muscle becomes tight, overworked, or inflamed due to prolonged sitting, repetitive exercise, or poor movement mechanics, it can place pressure on the sciatic nerve and produce pain that travels down the leg.

Common symptoms include deep buttock pain, discomfort while sitting for long periods, pain during prolonged driving, tenderness around the hip, and radiating pain along the back of the thigh. Unlike lumbar disc-related sciatica, Piriformis Syndrome often occurs without significant low back pain, although the symptoms can be very similar. A thorough examination by a physical therapist is important to distinguish between these conditions and determine the most effective treatment approach.

Physical therapy focuses on relieving pressure on the sciatic nerve while correcting the underlying biomechanical factors. Treatment typically includes manual therapy, stretching of the piriformis muscle, strengthening of the hip and core muscles, movement retraining, and advice on posture and activity modification. Individualized rehabilitation helps restore normal movement, improve hip stability, and reduce the likelihood of recurring symptoms.

If you experience persistent buttock pain or pain radiating down the leg, do not assume it is simply a spinal problem. Early evaluation by a physical therapist can identify the true source of your symptoms, guide appropriate rehabilitation, and help you return safely to work, exercise, and everyday activities with greater comfort and confidence.
